Submitting an appointment request through the Site does not create a doctor-patient relationship until an appointment is confirmed by our office.
Appointment requests
Appointment requests submitted online are not confirmed until a member of our team contacts you by phone, text, or email. Please do not submit health information through the Site.
Communications and SMS consent
By providing your phone number and/or email address, you agree that North Billerica Smiles may contact you by phone, email, and SMS text message regarding your request, appointments, and our services. Message frequency varies. Message and data rates may apply. Reply STOP to any text to opt out and HELP for assistance. Consent to receive text messages is not a condition of purchase. Our full SMS Terms and Privacy Policy are incorporated into these Terms.
Membership plans
Smile Club membership plans are in-office discount plans, not insurance. Plans are valid for one year from sign-up, carry a one-time enrollment fee (waived if paid in full), and are non-transferable. Periodontally involved patients must enroll in the periodontal plans. Full plan terms are provided at enrollment.
Intellectual property
All content on the Site, including text, graphics, logos, and images, is the property of North Billerica Smiles or its licensors and may not be reproduced without written permission.
Third-party links
The Site may link to third-party websites, including financing partners and social media. We are not responsible for the content or privacy practices of those sites.
Disclaimer and limitation of liability
